Every four years, Mecklenburg County conducts a Community Health Assessment to identify, describe and prioritize issues affecting the public health of our community. As part of the 2013 assessment, we welcome you to participate in a brief health opinion survey. The survey is available in English and Spanish and should take 5-10 minutes to complete. Responses are completely anonymous.

We are interested in hearing from as many people as possible across the county and ask that you forward this email with the survey link to your friends, colleagues and co-workers. A paper version of the survey is available and we would be willing to deliver copies if you have clients or groups who might not have access to the online survey.

Thank you for providing us with input for our Community Health Assessment.
